Evaluation of Protein-osmolality Ratio And Protein-creatinine Ratio from a Random urinalysis Sample for the Quantitative Assessment of Proteinuria

Abstract
Background:
Quantitative assessment of proteinuria is valuable for the diagnosis and evaluation of patients with renal disease. The collection of a 24-hour urine sample to assess proteinuria is inconvenient and often unreliable. The authors evaluated
whether a
urinary protein-creatinine ratio(mg/dl/mg/dl) and a urinary protein-osmolality ratio(mg/L/mOsm/kg) in a random urinalysis sample predicts the 24-hour urine protein level.
Methods:
We compared both of these ratios with concurrent 24-hour urine protein level respectively in 75 proteinuric patients and 19 patients without proteinuria.
Results:
Both of them correlated with 24-hour urine protein levels (r=0.88, p<0.001 for the protein-creatinine ratio and r=0.86, p<0.001 for the protein-osmolality ratio). We estimated that the normal urinary protein-creatinine ratio and
protein-osmolality
ratio were less than 0.2 and 0.3 respectively based on the equation of log-regression lines for the protein levels versus respective ratios. With a level less than 0.2 considered normal for the protein-creatinine ratio, 22% of normal patients had
a
ratio greater than 0.2(specificity ; 78%) and 8% of pathological proteinuria patients had a ratio less than 0.2(sensitivity ; 92%). The sensitivity and specificity of the normal urinary protein-osmolality ratio level less than 0.3 were 90% and
70%
respectively. The protein level of more than 3.0g/day is indicated by a urinary protein-creatinine ratio of greater than 2.8 or urinary protein-osmolality ratio of greater than 4.9(sensitivity & specificity for protein-creatinine ratio were 88% &

Conclusions:
The authors conclude that both protein-reatinine ratio and protein-osmolality ratio from a random urinalysis sample are reliable and useful indices for the detection of abnormal proteinuria.
